Help us learn more about your child and how we can best prepare him or her for the MSA environment. Complete the details below to schedule a visit and assessment test:

Qualifications

Grade One- children who graduated from Prep and are 6 years old by October
Grade Two- children who graduated from Prep and are 7 years old by October
Grade Three- children who graduated from Prep and are 8 years old by October
Grade Four- children who graduated from Prep and are 9 years old by October
Grade Five- children who graduated from Prep and are 10 years old by October
Grade Six- children who graduated from Prep and are 11 years old by October

General Instructions to Parents

For Assessment
1. Accomplish the assessment request form.
2. An original NSO birth plus a clear copy
3. Pay the PHP 500.00 assessment fee. An assessment permit will then be issued.
4. Bring this permit on the day and time of assessment at MSA and present this to the administrative office.
For Enrollment
1. Accomplish and submit the Student application form. Write in BLOCK LETTERS.

*Additional Requirements for Transferees

Latest progress Report or Report Card (Form 138)
Two pieces of 2″ x 2″ photo of the child
One piece of 2″ x 2″ photo of the mother, father, or guardians
