Qualifications

Toddler- Children who are 2 years old by October
Nursery- Children who are 3 years old by October
Pre-Kinder- Children who are 4 years old by October
Kinder- Children who are 5 years old by October

General Instructions to Parents

For Assessment
1. Accomplish the assessment request form.
2. Submit a copy of the birth certificate
3. Pay the PHP 500.00 assessment fee. An assessment permit will then be issued.
4. Bring this permit on the day and time of assessment at MSA and present this to the administrative office.
For Enrollment
1. Accomplish and submit the Student application form. Write in BLOCK LETTERS.

*Additional Requirements for Transferees

Latest Progress Report or Report Card (Form 138)
Two pieces of 2″ x 2″ photo of the child
One piece of 2″ x 2″ photo of the mother, father, or guardians
